Referral Clerk
CHCS, Inc. is seeking a detail-oriented and organized Referral Coordinator to manage and track patient referrals. This role is responsible for ensuring timely and accurate processing of referrals, supporting patient access to specialty care, and maintaining communication between patients, providers, and external facilities. The ideal candidate will be proactive, compassionate, and able to manage multiple tasks in a fast-paced healthcare environment.
Key Responsibilities:
Process and coordinate all assigned routine outgoing patient referrals in accordance with CHCSS, Inc. policies and procedures.
Assist with urgent and stat referrals as needed to ensure timely patient care.
Track referral status, including appointments kept and missed, and follow up appropriately.
Notify patients of referral appointments via phone or mail.
Communicate with providers regarding missed appointments, delays, or barriers to care.
Document patient concerns and referral updates in the EMR system.
Coordinate with specialty care providers to ensure required documentation is submitted and received.
Monitor receipt of consultation reports and maintain accurate referral records.
Assist patients in overcoming barriers by utilizing community resources.
Obtain insurance authorizations for specialty services when required.
Collaborate with team members to ensure efficient workflow and coverage of referral duties.
Qualifications:
High school diploma or equivalent required; associate degree preferred.
Previous experience in a healthcare or administrative setting preferred.
Familiarity with electronic medical records (EMR) systems.
Knowledge of medical terminology and referral processes is a plus.
Strong organizational, communication, and multitasking skills.
Ability to maintain confidentiality and handle sensitive information.
