Join our mission-driven team supporting PACE (Program of All-Inclusive Care for the Elderly) participants in Fort Gratiot, MI. This part-time role is perfect for pharmacists passionate about geriatric care and interdisciplinary, patient-centered practice.
What You'll Do:
Review and manage medication regimens for older adults with complex needs
Conduct medication reconciliations and chart reviews
Identify, resolve, and prevent medication-related problems
Participate in interdisciplinary team (IDT) meetings and care planning
Ensure compliance with CMS, state pharmacy laws, and PACE standards
Provide education to participants, caregivers, and healthcare team members
Requirements:
PharmD from an accredited program
Active Pharmacist License & Controlled Substance License
2+ years experience in geriatrics, long-term care, or related setting
Willingness to travel to Michigan cities of Alpena, Ft. Gratiot, and Mt. Pleasant
