JOB SUMMARY

The Skilled Nursing Occupational Therapist evaluates and treats short-term rehab patients and long-term care residents to improve or maintain function and independence. This role develops individualized, patient-centered therapy plans, supports admissions and discharges, and monitors changes in condition to reduce functional decline.

The Occupational Therapist communicates progress with patients, families, and the care team, while contributing to program development, professional growth, and departmental goals to enhance patient outcomes.

ESSENTIAL FUNCTIONS

Evaluate and treat short-term rehab patients and LTC residents; develop and document individualized plans of care in the EMR.

Monitor progress, complete re-evaluations, and adjust treatment plans to achieve therapy goals.

Collaborate with OTAs and the interdisciplinary team to ensure coordinated, high-quality care.

Complete timely, accurate documentation, including daily notes, screenings, and required reports.

Conduct routine and as-needed resident screenings to support function, independence, and quality of life.

Communicate effectively with patients, families, and care teams regarding plans of care and progress.

Participate in admissions, discharges, care planning, and team meetings.

Provide supervision of OTAs, aides, and students in accordance with regulatory standards.

Maintain professional standards, licensure, and compliance with all applicable regulations and organizational policies.

Demonstrate service excellence, strong time management, and positive teamwork.

Support operational needs, including flexible scheduling, weekend/holiday rotations, and cross-coverage as needed.

Responsible for the assessment of short-term rehabilitation patients and LTC residents through a review of the medical history, administration of standardized and informal test measures, identification of deficits and determining prognosis in order to establish an individualized, patient-centered plan of care and documentation in the approved EMR.

Abide by the National Board of Occupational Therapy (NBCOT), Wyoming Occupational Therapy (WYOT) and CCH’s Code of Conduct by respecting the rights, knowledge, and skills of colleagues and other healthcare professionals to maximize patient/resident outcomes.

The Occupational Therapist shall adhere to Campbell County Health’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.

Must be free from governmental sanctions involving health care and/or financial practices.

Perform all other duties as assigned.

JOB QUALIFICATIONS

Education

Graduate of an approved/accredited occupational therapy school.

License

Must currently be licensed in, or be eligible to be licensed, to practice occupational therapy in the state of Wyoming. xsgimln
