Plaza de la Raza Child Development Services, Inc. operates a Head Start and Early Head Start program that provides comprehensive integrated services to children from birth to five years of age and is currently accepting bids for a Registered Nurse to provide support that support the health and well-being of children and families. Services to be provided from July 1, 2026 – June 30, 2027.
Job Description:
• Review, evaluate, and interpret child health and nutrition screening forms to identify and address health concerns.
• Assist with medication monitoring and conduct medication reviews.
• Document all reviews directly in the database system, ensuring accuracy, timeliness, and completeness in alignment with the documentation standards required.
• Complete and or review Individualized Health Care Plans for children as needed.
• Collaborate with staff on the completion of Individualized Nutrition Health Care Plans.
• Provide staff training on medication administration
• Conduct follow-up and provide education to parents of children with identified health conditions or missing information.
• Monitor medication uses and assist with regular medication reviews.
• Attend Health and Mental Health Advisory Meetings.
• Participate in Multidisciplinary Team Meetings
