About This Job
The Reintegration Mentor provides ongoing support and guidance to male adolescents transitioning from treatment all across the state of CT, serving as a role model and advocate. The role involves facilitating treatment plans, coordinating appointments and community activities, and offering aftercare for up to 12 months post-discharge. The position requires engaging with youth both in the facility and in the community, working collaboratively with staff and families, and maintaining connections to promote successful reintegration. Flexibility in hours, including evenings and weekends, and the ability to work independently are essential. Overall, the position aims to support adolescents’ ongoing development and stability as they transition back into the community.

Responsibilities and Duties

Engage with youth in the facility

Maintain relationships with facility staff on behalf of the adolescents

Facilitate ARISE Curriculum

Advocate for youth and their families

Attend court or providers meetings

Play a significant role in the client’s treatment team

Work on a flexible schedule, which includes weekend and evening hours

Share on‑call hours with other staff

Develop knowledge base of community resources across Connecticut

Maintain connection with youth and family throughout treatment and beyond

Perform aftercare for RESTS clients for one year

Requirements and Qualifications

High school diploma or equivalent with lived experience, and experience, training, and a history of working with adolescents

Comfortable traveling throughout the state of CT

Understanding unique issues and needs of adolescents

Flexible hours, weekends and evenings

Valid driver’s license to utilize a company vehicle during work hours

Skill in advocacy, crisis intervention and ability to function independently

Ability to speak Spanish is a plus

Schedule and Rate of Pay

Salary pay range is $60,000-$65,000 (actual pay is determined based on job‑related factors such as skills, education, training, and performance)

Schedule will include 35 hours per week

Some evenings may be required based on family schedules

Schedule flexibility will be required to ensure the youth and family’s needs are met
