Social Worker – Part Time
Concordia at Villa St. Joseph is seeking a compassionate and motivated Social Worker – Part Time to join our care team. This is a fantastic opportunity for a professional who is passionate about serving the geriatric population and is looking for a role with flexibility, growth potential, and meaningful work.
Pay:
$21.00 – $25.00 per hour
Key Responsibilities
Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.
Participate in interdisciplinary care conferences and update resident care plans; involve families in care planning, offer support, and provide guidance on community services.
Coordinate follow‑up services and provide resources to residents and families for discharge planning.
Complete timely and accurate notes, assessments, and reports in compliance with regulations.
Qualifications
Bachelor’s Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.).
At least one year of supervised social work experience in a health care setting preferred but not required.
Strong documentation skills, communication skills with residents, families, staff, and community partners, and ability to work independently and as part of a team.
