Shifts are Monday to Friday, 8:00 AM to 4:30 PM (7.5-hour shifts with a 1-hour lunch). Weekend and holiday coverage may be required on rare occasions with manager approval.
Job Description
The Inpatient Case Manager will focus on discharge planning while working collaboratively with an interdisciplinary team and patients. The role requires independent functioning within a Med-Surg or Telemetry unit, alongside a Social Worker. Key responsibilities include assessing patient needs, making referrals, and ensuring smooth transitions from hospital to home or other facilities.
Responsibilities
Conduct discharge planning for patients, including creating referrals to community resources.
Round on the unit with physicians and interdisciplinary team members.
Manage a caseload of 17-25 patients, ensuring effective transitions to home, community, or skilled nursing facilities.
Provide assistance with obtaining housing and necessary medical equipment or services.
Utilize EMR software (Allscripts) for documentation and case management tasks.
Required Experience / Certifications / Licensure
Minimum of 1 year of inpatient case management experience required; prior critical care experience preferred.
Current BLS certification.
PRI and SCREEN certifications (can be obtained online, cost approx. $300 each).
Familiarity with CarePort, MCG, and/or InterQual preferred.
