RN Case Manager: Care Coordination & Discharge Planning

Origin Travel Nurses is seeking an RN Case Manager in Chicago, IL. This role requires a current Illinois RN license and at least 2 years of case management experience. The RN Case Manager will coordinate care management, develop individualized care plans, and advocate for patients' needs.
