Inpatient Intake Coordinator

The Intake Coordinator is responsible for facilitating the intake and assessment process for individuals entering the Crisis Stabilization Program. This role involves conducting initial assessments, coordinating admissions, and ensuring a smooth transition for individuals in crisis. The Intake Coordinator collaborates with the crisis intervention team, community agencies, and individuals seeking assistance to promote timely access to crisis stabilization services. This position will assist the Psychiatric Hospital and Social Services Department as needed and may provide clinical services in addition to these other duties.
This position is part-time day shift up to 20 hours a week.
Duties / Responsibilities

Receive and review referrals for Crisis Stabilization services from various sources.
Collaborate with emergency services, community partners, and healthcare providers for seamless transitions.
Communicate effectively with Crisis Intervention team members, clinicians, and external agencies.
Offer immediate support and empathy to individuals and their families during the intake process.
Collaborate with Peer Support Specialists and Crisis Intervention team to address immediate needs.
Ensure individuals are informed about available services and engaged in the crisis stabilization process.
Conduct comprehensive assessments of individuals entering the Crisis Stabilization Program.
Gather relevant information on mental health and substance use history, current crisis, and immediate needs.
Collaborate with the crisis team to determine the appropriate level of care and intervention.
Coordinate pre-admission medical clearance if needed.
Accurately and objectively apply admission and exclusion criteria for inpatient admission.
When referring to the Psychiatric Hospital, staff the case with the admission provider.
Collect and communicate pertinent admission information.
Assist front office by obtaining pertinent financial and/or insurance information as needed.
Assist patients with self-pay payment plans and/or presumptive eligibility applications.
Provide clinical services in the form of ANSA's and follow up services post discharge or denial of admission.
Follow up with individuals post discharge to assess ongoing needs and coordinate appropriate referrals.
Facilitate connections to community resources and longer-term mental health services.
Provide information on aftercare plans and support for continued recovery.
Maintain accurate and confidential records of intake assessments and admissions.
Ensure compliance with all relevant privacy and confidentiality regulations.
Assist Social Services Department as needed.
Other duties as assigned.
Physical Requirements

Occupational Exposure Risk Classification: The normal work routine involves minimal exposure to blood, body fluids or tissues.
The normal work routine has a minimal ergonomic risk for repetitive motion impairment.
The position may require a minimal amount of physical exertion (bending, lifting, twisting) in the work area.
The position requires the ability to engage with patients who may become violent and to participate in physical patient holds using approved techniques when necessary.
Required Qualifications

Education: Bachelor’s degree in psychology, Social Work, Counseling, or Mental Health related field. Master’s Degree preferred.
Experience: Prefer two (2) years in Crisis Intervention, mental health or related field.
Certificate or Professional License:
CPI (Crisis Prevention Intervention) certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
BLS (Basic Life Support) and First Aid certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
Valid driver’s license and active auto insurance coverage.
Satisfactory results from criminal and driving record background checks.
