Introduction
The LPCMH is a mental health professional who provides counseling, evaluation, intervention, and case management services to those who are suffering mental, social, familial and/or medical issues. Examine the social and environmental factors impacting the client’s life.
Job Summary
The incumbent conducts mental health diagnostic evaluations for children, adolescents and adults and provides treatment accordingly. Completes documentation in the EMR, participates in clinical and peer consultation groups and communicates professionally with medical staff and community partners.

Key responsibilities include:
Assists in the assessment, planning, arranging and implementing of psychosocial programs e.g. domestic violence, parenting skills, etc.
The LPCMH functions as an integral member of a team delivering a variety of services to patients referred by a primary care provider.
Engages in population-based education, prevention and early intervention with patients, and participates in chronic disease management, including psychosocial assessments.
Evaluates patients and provides counseling and crisis intervention as required.
Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs.
Counsels clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
Conducts specific targeted counseling programs such as P.C.I. T. in dealing with the need for child/parent interventions.
Refers patient, client, or family to community resources to assist in recovery from mental or physical illness.
Collaborates with other professionals and remains current with new behavioral health approaches and strategies.
Performs other duties and responsibilities as assigned.
Requirements
Education and/or Experience
LPCMH or LCSW licensed therapist.
Master’s in social work.
3‑5 years of experience in private practice or public health setting.
Language Skills
Bilingual (English/Spanish) skills preferred.
Knowledge, Skills, and Abilities
Knowledge of a wide area of social service programs; counseling and intervention practices; standard use and care of medical equipment and supplies; EMR system and medical record documentation requirements; state/federal laws related to the release of health care information.
Skills in time management, problem solving, multitasking, prioritizing; initiating appropriate crisis interventions and emergency response; organizing and developing patient information and training materials; dealing with large amounts of information in an organized manner; using computer and medical records software.
Abilities to interact in a multicultural environment; determine and assess patient needs; build rapport, trust, and patient-centered care; make sound psychological assessments; communicate effectively and develop rapport.
Equipment Operated
Standard office equipment: copier, fax, EMR, scanner, etc.
Working Conditions
Medical offices in Health Center. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and patient responses.
Physical/Mental Requirements
Work involves standing, walking, kneeling, bending, grasping, manipulating, and squatting. Ability to lift up to 50 pounds. Walking or standing may exceed two (2) hours per day. Ability to work in a standard medical office setting, including computer use; ability to read computer screens and printed materials; hearing and speech required for communication in person and over the phone; must be able to attend meetings at various sites; stress can be triggered by workload; the work is at times sedentary.
