Job Description
SBH Health System is seeking a compassionate and organized
Pediatric Collaborative Care Coordinator
to support youth ages 12–21 across our three pediatric ambulatory care sites. This role ensures timely access to behavioral health services, smooth care transitions, and strong collaboration with
Pediatricians, Social Workers, and Behavioral Health providers .
Responsibilities
Coordinate mental health referrals, follow‑up, and care transitions across all pediatric sites
Partner closely with Pediatricians and Social Workers to support integrated behavioral health care
Provide youth‑centered education on mental health conditions, coping skills, and treatment plans
Assist with mental health screenings and ensure timely follow‑up for elevated scores
Support crisis response, safety planning, and linkage to urgent services
Connect families to community mental health, school‑based, and social service resources
Maintain accurate documentation and track high‑risk youth for follow‑up
Participate in interdisciplinary case conferences and quality improvement initiatives
Requirements
QUALIFICATIONS:
Education Required - HS Diploma or Equivalence
License/Certification/Registration(s) Required - Graduate from an Approved Medical Assistant Program
Experience/Skills Required
Previous experience in a medical setting or 2 years relevant work experience preferred
Excellent communication/ interpersonal skills
Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
