Job Responsibilities

Assesses the patient’s physiologic health status, conducts patient interviews, reviews charts, and documents findings.

Assesses gastrointestinal, cardiovascular, respiratory, renal, neurological, and pain status.
Determines mobility, sensory deficits, prostheses use, skin condition, and adjusts the plan of care accordingly.
Communicates the patient’s physiologic status and plan of care.

Assesses the patient’s psychosocial health status.

Elicits perceptions of medical
ursing care and expectations.
Determines coping mechanisms, knowledge level, and ability to comprehend.
Identifies cultural/ethnic requirements to reduce anxiety and ensure satisfaction.
Documents psychological status and care plan; refers to support group, counseling services, and social services.

Identifies patient outcomes.

Develops criteria for measuring outcomes.
Identifies actual and potential patient problems.
Determines teaching needs based on psychosocial and developmental assessment.
Sets individualized outcome statements and goals.

Formulates the plan of care and establishes priorities.

Identifies care activities and prioritizes them.
Co‑coordinates cost‑effective use of supplies, equipment, and medication.
Documents and collaborates with physicians and team.

Implements the plan of care.

Maintains vigilance over patient safety (e.g., side rails, call lights).
Administers prescribed treatments and uses equipment properly.
Provides emotional support and maintains calm environment.
Updated documentation of treatments, activities, and patient responses.
Responds rapidly and effectively in emergencies.
Participates in discharge planning and coordination of referrals.

Evaluates care for patient outcomes.

Measures effectiveness, performs variance analysis, and documents findings.
Implements system improvements.

Collaborates with other care team members.

Coordinates with physicians on patient care plans.
Delegates assignments to associates and ensures completion.
Provides updates to Patient Care Manager and physicians as needed.

Performs grade I‑IV decubitus care, skin care, wound dressing, and related procedures.
Administers medications safely and accurately.

Identifies medication action, dosage, side effects, and implications.
Prepares local solutions and IV drips.
Documents administration per policy.
Educates patients on drug–food interactions.

Participates in patient and family education.

Explains condition and provides assessment data.
Utilizes available teaching resources.
Facilitates discharge planning.
Evaluates teaching effectiveness by feedback.

Physical Demands

Constant: standing, walking, stooping, bending, stretching, lifting, pulling, pushing, turning.
Frequent: lifting objects 10–150 lbs, pushing objects up to 200 lbs.
Occasional: sitting; lifting with assistance objects >150 lbs and carrying up to 50 lbs.
Job Qualifications

Graduated from an accredited School of Nursing; Bachelor’s Degree preferred.
Must be enrolled in an accredited program within 24 months of employment if hired after Sep 1, 2010, and obtain a BSN within five years.
Current license to practice as a Registered Professional Nurse in New York State.
BCLS and ACLS preferred.
