Broadview Health & Rehabilitation in Greeley, CO is a post-acute care facility offering long-term and short-term care, including geriatric services. We provide a stable and collaborative in-house therapy team with excellent programs for our geriatric clientele.

Full Time : Flexible schedule with work-life balance

Pay : $43 - $50 /hour based on experience

Qualifications

Licensed Occupational Therapist

Open to applicants of all experience levels

Key Responsibilities

Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.

Utilize standardized assessments and evidence-based practice to support clinical interventions.

Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.

Administer various procedures as part of the rehabilitation plan including but not limited to: ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.

Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.

Evaluate treatment outcomes for effectiveness and modify services relative to patient evaluation; refer as needed.

Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.

Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.

Provide comprehensive discharge summary of services per regulatory and clinical requirements.
