What You'll Do
Provide skilled nursing care during home visits
Assess patient needs and help create individualized plans of care
Administer medications, treatments, and wound care as ordered
Monitor patient progress and communicate changes with the care team
Educate patients and families with patience, empathy, and encouragement
Support patients in maintaining comfort, independence, and quality of life
Complete documentation accurately and in a timely manner
Collaborate with physicians, therapists, aides, and other team members to ensure coordinated care
What We're Looking For
Current and active LPN license in Indiana
Minimum of 1 year of nursing experience
Home health experience required
Strong clinical judgment and assessment skills
A caring, dependable, and patient-centered approach
Excellent communication and time-management skills
Current CPR certification
Valid driver’s license, reliable transportation, and auto insurance
