Prior Authorization Pharmacist – Role Overview
The Prior Authorization (PA) Pharmacist is a licensed clinical professional responsible for reviewing, evaluating, and making coverage determinations for medication prior authorization requests in alignment with company policies, clinical guidelines, and regulatory requirements. This role ensures the safe, appropriate, and cost-effective use of medications while supporting providers, members, and internal teams through timely, accurate clinical decision-making.
Success in this role requires strong clinical judgment, in-depth knowledge of pharmacy benefit management (PBM), and the ability to apply evidence-based criteria to complex medication requests. The PA Pharmacist collaborates cross-functionally to support operational efficiency, compliance, quality assurance, and continuous improvement within a fast-paced PBM environment.
Key Responsibilities:
Review prior authorization requests for formulary and non-formulary medications using clinical guidelines, FDA-approved indications, utilization management policies, and evidence-based criteria.
Make approval, denial, or modification determinations within required turnaround times and licensure scope.
Accurately document clinical rationale, request details, and outcomes within internal systems.
Communicate determinations professionally with healthcare providers, pharmacies, and internal stakeholders.
Recommend clinically appropriate alternatives, step therapy substitutions, or cost-effective treatment options when applicable.
Collaborate with Clinical Services teams to ensure consistency, quality, and compliance in PA determinations.
Support escalated clinical inquiries, benefit interpretation, and policy clarification.
Maintain compliance with federal and state regulations, accreditation standards, and internal company policies.
Participate in policy reviews, formulary updates, and utilization management strategy development.
Provide clinical expertise for formulary management, analytics, operations, and client-specific initiatives.
Assist in staff training related to clinical criteria, medication classes, and PA workflows.
Support audits, quality assurance initiatives, and process improvements to enhance efficiency and accuracy.
Required Qualifications:
Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an accredited institution.
Active, unrestricted U.S. pharmacist license.
Experience in prior authorization, utilization management, PBM, managed care, or payer pharmacy operations.
Strong knowledge of clinical guidelines, specialty medications, formulary management, and drug therapy.
Excellent analytical, documentation, and clinical decision-making skills.
Proficiency with pharmacy benefit systems, PA platforms, and electronic documentation systems.
Strong communication skills, both written and verbal.
Ability to manage multiple priorities in a deadline-driven environment.
High integrity and ability to manage confidential clinical information.
Preferred Qualifications:
Experience with specialty medications, biologics, or complex therapy classes.
Previous PBM, health plan, or payer experience.
Background in formulary management, utilization management strategy, or clinical criteria development.
Familiarity with claims systems, pharmacy informatics, or PA automation tools.
