
Pharmacist - Clinical

Job Summary
Under limited supervision, provides clinical pharmacy services including primary care services, consultative services, and drug information services to patients and health care providers to deliver high quality care. Reviews prescriptions as written by authorized prescribers to determine that orders are safe and appropriate for the specific patient; contacts authorized prescribers when necessary to clarify vague or illegible prescriptions. Fills prescriptions by dispensing or compounding medication according to established procedures. Interviews patients for medication history and provides counseling on current drug therapy, identifying those who require additional teaching and supplying educational material. Works collaboratively with other health care providers to coordinate patient education. Assigns, educates, trains, and supervises the work of support personnel in medication preparation; ensures accuracy and compliance with policies and procedures. Maintains an inventory of medications and supplies, ensuring proper storage, regulatory compliance, and documentation. Provides formal patient and staff educational programs and conducts area inspections to ensure drug availability and proper storage. Provides clinical pharmacy services in therapeutic drug monitoring, anti‑coagulation service, nutritional support, investigational drug services, hematology/oncology pharmacy, medication use evaluation, pain management service, and adverse drug reaction reporting, adhering to system, hospital, and departmental policies and objectives.

Qualifications
Doctor of Pharmacy or Bachelor of Science in Pharmacy from an accredited College of Pharmacy, or completion of foreign equivalency examination.
Registered Pharmacist and Controlled Substance licensure in the State of Michigan obtained within three months of entry into the job.
