Clinical Social Worker - Home Health
Care IV Home Health is now hiring a PRN Clinical Social Worker for our Little Rock territory. Seeking a Social Worker to visit our home health patients to assess their medical social services needs & guide them through the process in conjunction with their physician's orders.

Duties include, but are not necessarily limited to:
Provide rehab and supportive casework geared towards restoring patients to their optimal level of social, emotional & health wellness
Help patients utilize appropriate family & community resources
Refer patients to appropriate health & welfare agencies
Consult & coordinate services with physicians and other members of the patient's care team
Prepare clinical progress notes & incorporate into clinical record
Participate in discharge planning & in-service programs
Required education & experience includes:
Master's degree in social work from a college accredited by the Council of Social Work Education
Arkansas social worker license that is valid and in good standing with the licensing board
6 months of professional social work experience in a skilled medical environment such as home health, hospital, rehab facility, clinic or related
