Position Overview
Case Manager/Wraparound Facilitator at Home Base, Inc. is responsible for providing wraparound services to families, coordinating comprehensive support, and ensuring optimal outcomes for youth and families in the behavioral health program.
Primary Responsibilities
Provide wraparound services to families with a limit of up to 15 members.
Coordinate a comprehensive set of supports, resources, and strategies for each member and family.
Utilize a strengths-based approach to plan, coordinate, collaborate, and facilitate team meetings with the youth, family, and team members.
Ensure services and clinical treatment modalities augment each other for optimal outcomes.
Work with the MCO care manager and family to identify natural supports and other community resources.
Lead the planning team through engagement and team preparation, initial plan development, implementation, and transition planning.
Administer the Child and Adolescent Needs and Strengths assessment tool.
Monitor plan implementation, ensuring families receive the services identified in the plan.
Initiate and coordinate discharge and after-care planning.
Provide linkage and referral to services and supports as identified in the plan.
Conduct home visits with youth and/or family in accordance with the individualized plan.
Identify and develop formal and informal services/resources available to youth, family, and community.
Identify service gaps and collaborate with community stakeholders and existing service providers for resolution.
Facilitate the development and implementation of an individualized transition plan for members reaching the waiver’s maximum age limit.
Ensure documentation is thorough, accurate, and timely.
Collect and compile pertinent data as required.
Participate in weekly and monthly supervision and coaching sessions.
Maintain client records in compliance with organizational goals, objectives, and regulatory guidelines.
Qualifications
Bachelor’s Degree in a human services field required.
2-4 years of experience in case management and/or an outpatient/ inpatient behavioral health setting preferred.
1-2 years of experience leading teams preferred.
Strong communication skills and ability to demonstrate experience.
Proficient computer skills; comfortable with Outlook, Word, Excel, and PDF.
Valid driver's license and reliable transportation.
Pass an NCIC/CIB background check and an APS/CPS background check.
Provide three professional references.
Experience with virtual platforms.
Professionalism and customer service skills essential.
Excellent written communication skills.
Ability to work independently and without supervision at times.
Availability to work evenings and weekends.
Time management skills required.
Physical Requirements
Must be able to walk up and down stairs.
Must be able to operate a motor vehicle and comply with all applicable motor vehicle laws.
Must be able to lift 5-50 pounds.
Must be able to kneel, bend, squat, and stand.
