Hospice Social Worker
Salary: $65-78k/yr BOE
Schedule: M-F| 8:00AM-5:00PM +(on call)
Location: Dayton, Ohio and surrounding areas
Requirements of the Hospice LSW role:
Licensed Social Worker (LSW) in the state of practice
Bachelor's Degree from a school of Social Work accredited by the Council of Social Work Education, Masters degree preferred.
One year of social work experience in a healthcare setting preferred
Current Driver's License, vehicle insurance, and access to dependable transportation.
About the Hospice MSW role:
Provide medical social services to the patient and/or family according to the Plan of Care.
Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient's care.
Participate in IDG meetings and activities and the development of the patient plan of care.
Observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and response to treatment.
Maintain and submit written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes.
Confirm weekly, the scheduling of visits with the patient's case managers to facilitate coordination of other staff visits.
Assess the patient's and family member's ability to cope with the patient's passing.
Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems.
