The Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.
Primary Responsibilities
Provides high quality clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other healthcare team members, following all applicable infection control procedures.
Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable).
Actively participates in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending patient case conferences, and effectively communicating with the patient, family, physician, other clinicians, agency management staff, case coordinators, and community case managers (as applicable).

Required Qualifications
Current and unrestricted LPN licensure in the state of practice.
Current CPR certification.
Current driver's license and vehicle insurance; access to a dependable vehicle or public transportation.
Ability to work flexible hours as required to meet identified client needs.
Ability to function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications
Home care experience.
Ability to manage multiple tasks simultaneously.
Able to work independently.
Good communication, writing, and organizational skills.
Pay is based on several factors including local labor markets, education, work experience, and certifications. The hourly pay for this role will range from $20.00 to $35.72 per hour based on full‑time employment. In addition to salary, benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution are offered (all benefits are subject to eligibility requirements).
