R42476### Summary:In addition to the Clinical Pharmacist I and II responsibilities, the Care Team Lead Pharmacist is expected to be an innovative leader and a source of expertise for their defined practice area.Under the supervision of the Pharmacy Leadership Team, the Clinical Pharmacist Care Team Lead will also be expected to serve as the content expert and liaison for pharmacy/patient care units and population-specific projects and tasks; serve as the care team representative and actively participate in Care Team meetings; assist in goal planning for the clinical program; assess and address educational needs of care team/staff members; establish standard tasks and expectations for the care team; be responsible for care team and departmental communication; and serve as a pharmacy preceptor for students/residents as appropriate/assigned.**### Job Responsibilities:**1) Provides comprehensive pharmaceutical care to all patients: Effectively utilizes the Clinical Information System (CIS) - Cerner to evaluate all orders for correct drug, strength, and dose specific to patient population, method of administration, allergies, dosing interval, drug interactions, duplication of therapy, stability/compatibility and formulary status. Maintains accurate documentation of all clinical interventions/clinical notes. Triages drug information requests appropriately. Demonstrates an acceptable safe rate of orders verified per hour and accurate clinical order verification as determined by the departmental standards. Performs all clinical duties assigned to pharmacist position which may include IV to PO dosing, Antimicrobial stewardship programs, etc. Supports and oversees distributive pharmacy services, which includes demonstrated competency with the various technology systems that support medication preparation and distribution.**2)**Leadership: Has oversight of care team practice area and provides supportive direction to the Pharmacy Staff as needed. Utilizing the principles of engagement, the leader serves as a liaison to the Care Team and Pharmacy Leadership Team communicating issues, areas of need and suggestions for improvement. The Care Team Lead serves as the primary resource for the training, orientation and education of employees, students and residents to their defined practice area. Oversees competency training and assessment for the staff in their defined area, including tracking software maintenance, etc. The Care team lead also participates in and leads practice area projects and tasks as required.**3)**Communication and Engagement: Maintains compliance in utilizing the following modes of communication as deemed appropriate per the BH and BMC Pharmacy policies and procedures: email, web paging, shift to shift communication logs, and verbal hand off. Understanding the avenues for additional decision support are required. Active participation in working committees, performance improvement teams, etc. to assist in the evaluation, planning, training, and implementation of such initiatives to improve clinical quality, safety, and/or regulatory compliance).**### Required Work Experience:**1) PGY-1/2 Residency or at least 3 years applicable experience**### Preferred Work Experience:**1) Demonstrated leadership experience strongly preferred**2)** MBA, MHA , PLA or completion of a Pharmacy Residency Administration Module also considered**### Skills and Competencies:**1) Possesses excellent interpersonal, verbal, and written communication skills**Education:**Bachelors Degree (Required)**Certifications:**Board Certified Pharmacotherapy Specialist - Board of Pharmacy SpecialistBoard of Pharmacy SpecialistBoard of Pharmacy Specialist, Licensed Pharmacist - OtherOtherOther**Compensation**Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system.
