Akkodis is hiring for a Behavioral Health Case Manager to work remotely for a 6 month contract!
Location: Remote, must be located within California

Pay: $55/hr
Job Summary
The Behavioral Health Case Manager supports members by assessing psychosocial needs, developing and coordinating care plans, and connecting members to appropriate medical, behavioral health, and community resources. This role works closely with members, families, providers, and internal care teams to promote recovery, wellness, and successful transitions of care.
Essential Functions
Conduct initial reviews of selected members and triage cases as appropriate.
Assess psychosocial, behavioral, and environmental factors that may impact health or recovery.
Develop and coordinate care plans in collaboration with members, providers, and plan staff.
Collaborate with members, family/caregivers, and internal and external partners to ensure understanding of and adherence to care plans.
Provide and coordinate referrals to professional, medical, behavioral health, and community resources.
Monitor member progress, evaluate outcomes, and implement interventions as needed.
Assist RN Case Managers and Concurrent Review Nurses with case management, discharge planning, and member transitions.
Support assessment and referral for mental health and behavioral health needs.
Perform community outreach for assigned members.
Support and guide members in self-advocacy efforts.
Document all activities accurately and timely in plan systems in accordance with established standards.
Track required data and prepare routine reports.
Knowledge, Skills, and Abilities
Required:
Ability to work independently and within an interdisciplinary team in a fast-paced environment.
Strong clinical judgment and decision-making skills.
Strong psychosocial assessment and analytical skills.
In-depth knowledge of mental health treatment, wellness, recovery, and strength-based approaches.
Knowledge of chronic and acute conditions and their impact on social functioning.
Knowledge of federal, state, and local regulations related to mental health and healthcare services.
Strong problem-solving, time management, and organizational skills.
Excellent verbal and written communication skills.
Strong interpersonal and customer service skills with diverse populations.
Ability to handle confidential information with discretion.
Proficiency in basic computer applications (Word, Excel) and ability to navigate multiple databases.
Preferred:
Knowledge of managed care principles and practices.
Knowledge of Medi-Cal or other government-sponsored healthcare programs.
Education and Experience
Required:
Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (MFT), or Licensed Professional Clinical Counselor (LPCC).
Minimum of three (3) years of experience as a social worker in a healthcare setting.
Preferred:
Experience working with Medi-Cal or other government-sponsored programs.
Experience in a managed care environment.
Licenses and Certifications
Required:
Current LCSW, MFT, or LPCC license issued by the California Board of Behavioral Sciences.
Valid California driver’s license and reliable transportation, or the ability to obtain transportation as required.
