Valid (HHA) Home Health Aide or (PCA) Personal Care Aide Certificate
Current Photo ID & Social Security card
Drug screen within last 6 months
Complete physical within last 6 months
Two reference letters (Personal or Professional)
HHA or PCA Certificate (Required)

Responsibilities
Assists patients by providing personal services such as bathing, dressing, and grooming
Provides social interaction
Reports any changes in the patient’s condition or incidents to RN Supervisor or therapist
