– Weekend Baylor Program
Rosewood Rehabilitation & Nursing Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Weekend Baylor Program team. Located in Schuylkill Haven, Pennsylvania, you will provide high-quality patient care to our residents under the supervision of our experienced healthcare team. Our Weekend Baylor Program offers a unique and rewarding opportunity with flexible weekend scheduling.
Responsibilities
Provide direct patient care, including administering medications
Monitor and report changes in patient condition to the registered nurse or physician
Collaborate with the interdisciplinary team to develop and implement individualized patient care plans
Maintain accurate and detailed records of patient care and treatments
Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
Foster a positive and respectful environment that promotes patient-centered care
Qualifications
Current LPN licensure in the state of Pennsylvania
High school diploma or equivalent required; some college or nursing program education preferred
Prior experience in a healthcare setting; previous experience in long-term care or rehabilitation setting a plus
Strong communication and interpersonal skills; ability to work effectively with patients, families, and healthcare team members
