Position Summary / Career Interest:

The Organ Transplant Nurse Coordinator plans, directs, and provides specialized nursing care with the primary emphasis on the treatment of individuals who have advanced or end‑stage disease and are in need of a transplant. Provides continuity and coordination of patient care; conducts patient and professional education initiatives; collaborates with multidisciplinary teams, community resources and agencies.
Responsibilities and Essential Job Functions

Demonstrate ability to provide comprehensive management of end‑stage organ diseases and oversee the patient's journey through the transplant process.
Demonstrate knowledge and ability to facilitate living donation/transplantation and coordinate the care of the living donor for evaluation and live donation. Identify appropriate live donor candidate upon referral and interact with appropriate staff.
Coordinate the care of the transplant recipient in the outpatient setting to achieve optimal physical, social and emotional rehabilitation. Identify and respond to recipient/family/legal guardian's educational, psychosocial and economic needs in the post‑transplant phase. Reinforce post‑operative teaching if applicable. Collect and review data pertinent to recipient's health and organ function. Communicate lab data and diagnostic results to transplant physician/healthcare provider. Implement changes in drug/treatment therapy as ordered by physician(s) and/or health care provider. Arrange for consultations, diagnostic procedures and hospitalization when indicated. Reinforce and facilitate needed well‑care (periodic mammography, pap smears and colonoscopy). Ensure recipient is screened annually for malignancy. Assess recipient's need for immunizations.
Monitor and coordinate care of the candidate awaiting organ transplantation. Develop and institute a teaching plan addressing transplant candidate/family/legal guardian's learning need. Maintain/ensure communication with patient, referring physicians, payers, other health care providers and transplant team members. Ensure all screening is obtained regularly per transplant center protocols. Maintain routine health maintenance and surveillance for malignancy during the waiting period. Exhibit ability to maintain and update candidate listing status per regulations. Identify the need for candidate re‑evaluation and initiate when appropriate. Provide and maintain comprehensive documentation of candidate progress.
Demonstrate proficiency and continual growth in the knowledge of transplantation. Review and integrate into practice the current professional literature in the field of transplant. Attend practice‑related conferences. Participate in professional transplant/procurement organizations. Examine current transplant‑related research. Share knowledge through mentoring/precepting newly hired staff. Demonstrate understanding and the ability to assume responsibility for individual judgments to achieve the highest quality of care for the transplant candidate, recipient, family and living donor.
Facilitate and coordinate evaluation of potential transplant candidates. Identify appropriate candidate upon referral and interact with appropriate staff. Identify requirements for candidate evaluation. Coordinate with multidisciplinary team the evaluation process for potential transplant candidate.
Assist with post‑transplant management including medications, complications, interventions and discharge planning. Collaborate effectively with multidisciplinary team. Monitor patient progress post‑transplant. Identify complications and consider interventions with the transplant team. Communicate recipient's status to referring physician, health care providers and payers. Identify and respond to the educational, psychosocial and economic needs of recipient/family/legal guardian during hospitalization. Educate recipient/family/legal guardian regarding care and responsibilities after transplantation.
Educate candidate/family/legal guardian about living donation options, evaluation process, regulations, organ allocation, waiting phase, transplant process, risks and benefits, including utilization of expanded donors, high‑risk donors, if applicable, post‑operative phase including immunosuppressive therapy and effects, potential research protocols of the transplant program, candidate/family/legal guardian responsibility throughout the transplant process.
Facilitate transplant process when organ becomes available to transplant candidate. Define and adhere to the current regulations and allocation policies. Communicate with regulatory bodies and collect the appropriate data necessary for decision‑making regarding organ acceptance or rejection for transplant. Share necessary information about the donor organ, including ABO compatibility, with the transplant surgeon/physician. Review potential recipient's current medical status to determine eligibility for transplant. Assess need for and obtain results of final cytotoxic cross‑match. Facilitate admission or transfer of patient to the transplant. Arrange any pre‑operative procedures required (e.g., dialysis). Notify transplant team of pending surgical procedure. Act as liaison between the donor and recipient transplant teams. Exhibit ability to remove transplant recipient from list per regulations. Provide outcome specifics to regulatory bodies for donor family follow‑up. Maintain communication with referring physician, health care providers and payers.
Identify and respond to psychosocial and economic needs of the transplant candidate/family/legal guardian during evaluation. Demonstrate knowledge of, and compliance with, policies and listing requirements. Inform and educate patient/family/legal guardian at time of listing. Ensure patient and referring physician are informed in writing of transplant listing, denial or postponement. Provide and maintain comprehensive documentation of evaluation process and make appropriate referrals to specialists.
Interpret serological results of the potential transplant candidate and their implications for transplant. Obtain recipient blood type, tissue typing and cytotoxic antibodies according to transplant center protocol. Identify possible co‑morbidities in the transplant candidate and evaluate as indicated. Identify absolute and relative contraindications to transplantation. Ensure patient understands risk and benefit of immunizations required prior to transplantation.
Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience

Bachelor degree, Nursing
2 or more years acute clinical experience
Required Licensure and Certification

Licensed Registered Nurse (LRN) – Multi‑State – State Board of Nursing Registered Nurse in State of Kansas
Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) or obtained within 14 days
Preferred Licensure and Certification

Must have current active multistate state RN license in good standing in state in which you live OR active single state RN license in good standing in the state you will be working with proof of application of a multistate RN license in the state in which you live.
Staff hired prior to 2/1/2024 are required to obtain a multistate state RN license from the state in which they live at the time of their next RN license renewal by 3/31/2026.
