Inpatient Case Manager - Per Diem
Fuller Hospital is a 109-bed licensed, private psychiatric facility located in South Attleboro, Massachusetts providing inpatient and outpatient behavioral health services to residents of Massachusetts and Rhode Island.
The Social Worker is an important member of the Fuller Hospital's inpatient multidisciplinary treatment team. This is a per diem position.
Responsibilities
Provide a comprehensive range of social work services to a variety of patients in an inpatient/partial hospital setting
Act as the primary contact with external providers and actively participate in each assigned patient’s treatment plan
Be the primary individual responsible for patient discharge and aftercare planning
Qualifications
Master’s Degree in Social Work, counseling, or another related human services field
Required LCSW, LICSW, LMFT, or LHMC (or eligibility to receive one of these licensures in the future)
Prior hospital social worker experience is preferred
