Must have candidates current name, no nicknames or abbreviations. Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Agency names are not accepted as the employer. Highest level of education listed, including month/year of graduation and degree achieved if applicable.
Skills Checklist
Completed within one (1) year of submittal for the specialty where candidate is submitted.
References
Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor. Reference must include full name, facility name, and dates of employment. Reference must include a breakdown of skills and strengths with a rating score. Letters of recommendation will not be accepted.
