Licensed Social Worker – North Shore, MA area including Lynn
We are seeking Licensed Social Workers (LICSW/LCSW) to support geriatric patients through a mix of infacility and patient home/facility visits.
Key Responsibilities
Participate in IDT assessments, meetings, and care planning,
Complete social work assessments and maintain timely, accurate documentation.
Provide psychosocial assessments, supportive counseling, and collaborate with health providers.
Support transitions to long‐term care and collaborate with Medicaid Specialists, SNFs, and caregivers.
Facilitate family meetings and connect families with community resources.
Assist with guardianship updates, health care proxy education, and insurance‐related documentation.
Review care plans with participants, guardians, and/or activated HCPs.
Complete authorizations for home care and other approved services.
Conduct home and SNF visits as needed; support palliative and end‐of‐life planning.
Communicate updates regarding disenrollment, LTC conversion, site transfers, and demographic changes.
Report allegations of abuse and complete required documentation.
Qualifications
Active Massachusetts LCSW or LICSW
Experience with geriatric populations
Strong assessment, communication, and documentation skills
