Role Description
This is a full-time, on-site Social Worker role located in Greater Boston. The Social Worker will be responsible for providing emotional and social support to residents and their families. Responsibilities include assessing residents' needs, developing individualized care plans, coordinating resources, and advocating for residents' rights. Additional duties include collaborating with interdisciplinary teams, facilitating activities that promote well-being, and ensuring compliance with healthcare regulations and standards.
Qualifications
Proficiency in case management, individualized care planning, and conducting psychosocial assessments
Strong communication skills, including empathetic listening and effective advocacy
Understanding of healthcare regulations, ethics, and confidentiality practices
Ability to collaborate with interdisciplinary teams and mediate conflicts when necessary
Master’s degree in Social Work (MSW) or related field, with current licensure (LCSW or LICSW preferred)
Experience working with elderly or rehabilitation patients is highly desirable
Strong organizational skills, with the ability to manage multiple tasks effectively
