Position
Care Coordinator (Full-Time / Non-Exempt) – Monday to Friday, 7:00 AM to 3:30 PM. Location: 309 Crutchfield Street, Durham, NC. Salary: $20.00 per hour.
Specialties / Licenses or Certifications
Bachelor’s Degree in a behavioral health field and at least one (1) year of behavioral health experience;
Bachelor’s Degree in any field and at least two (2) years of behavioral health experience;
High School Diploma or equivalent and at least four (4) years of behavioral health experience.
Job Summary
The Care Coordinator facilitates the quality and improvement of a person’s experience of care, which may include admission, discharge, outreach, engagement, recovery, transition, and case management.
Essential Duties & Responsibilities
Guest Care & Advocacy: Provide coordination of care services and resources internally and in the community; advocate for individuals’ needs and rights; assist with daily living activities and community linkages for education, support, and independence.
Compliance & Risk Management: Document required information and services, keep abreast of all regulations and Recovery Innovations policies, and adhere to compliance standards.
Cross‑Functional Collaboration: Coordinate or facilitate multidisciplinary team meetings, and partner with other provider agencies to support participants’ wellness.
Additional Duties: Perform other related duties as assigned.
Qualifications & Experience
Education: (Bachelor’s in behavioral health – 1 year experience; Bachelor’s in any field – 2 years experience; High School Diploma – 4 years experience)
Experience: Provide courteous assistance with a positive tone; work cooperatively in a team environment.
Computer Skills: Proficient in Microsoft Office Suite.
Communication: Excellent verbal and written communication, interpersonal skills, and customer service skills.
Organizational: Demonstrated ability to organize and maintain accurate records.
Training: Ability to pass de‑escalation training and mandatory driving policy if required.
Miscellaneous: Travel may be required. Minimum age 18 years; applicants must preserve confidentiality (HIPAA). Second‑Chance Employer: Must pass background check, substance abuse test, education verification, fingerprint clearance, and motor vehicle report (if applicable).
Physical Demands & Work Environment
Physical Demands: Close eye work, wrist and hand motions, sitting, standing, walking; may lift up to 20 pounds.
