Goodlettsville Clinic: 520 Rivergate Parkway, Goodlettsville, TN 37072. Clarksville Clinic: 267 Stone Crossing Drive, Clarksville, TN 37042. Proposed schedule: 3 days per week (Monday, Wednesday, Thursday) – 2 days at Goodlettsville, 1 day at Clarksville.

Responsibilities
Identify and assess patient’s clinical issues and health care needs
Record physical findings, and formulate plan and prognosis based on patient’s condition and the doctor’s plan of treatment
Evaluations and re‑evaluations of established patients
Refer patients to Physician for consultation or to specialized health resources for treatment when needed
Discuss case with Physician and other health professionals to prepare a comprehensive patient care plan
Perform pain management injections for specific diagnoses according to practice protocols
Prescribe/Order as medically indicated: Diagnostics, Therapeutics, Medication, Pain Management Procedures, DME
Demonstrate eagerness and ability to work independently as well as part of a team with flexibility and willingness to learn and take initiative on variety of tasks and projects
Support the vision and culture of the organization. Demonstrate personal commitment through active involvement in the performance improvement process
Perform all other duties as assigned
Skills and Abilities
Highly proficient with computers especially: EMR Software, Meaningful Use, Microsoft Word, Excel, Outlook
Ability to manage high levels of stress and capable of making sound difficult decisions
Structured, organized, consistent, pleasant, cheerful, yet professional personality, who will earn the respect of patients and co-workers immediately
Must be incredibly punctual. Great attendance, required
Qualifications, Education, and Experience (APRN)
Active, valid (in state of clinic) (APRN) Nurse Practitioner certificate, required
DEA Registration, preferred
Qualifications, Education, and Experience (PA‑C)
Must have a valid, unrestricted professional license in state of Indiana and be able to obtain clinical privileges to practice medicine
Physical Requirements
The physical demands of this position will include sitting and standing with occasional light to medium lifting. Ability to help lift patients and transport in wheelchair from clinic to parking lot. An employee must meet these requirements to successfully perform the essential duties of this job. Reasonable accommodation will be made in accordance with ADA rules and regulations.
