
VNA Clinical Manager – Boston Area

Overview
All Care is seeking an RN for the role of VNA Clinical Manager for our new Boston territory. This position is responsible for overseeing a team of clinicians and ensuring the delivery of high-quality patient care in a home health setting. This role manages clinical operations, supports staff performance, and ensures compliance with all regulatory standards while coordinating patient services to achieve optimal health outcomes.

This position follows a Monday–Friday schedule, with four days based in our Lynn office and one day working remotely.
Essential Duties & Responsibilities
Supervise and support a team of nurses and therapists
Review, approve, and oversee individualized patient care plans
Ensure compliance with federal, state, and accreditation standards (e.g., Medicare/Medicaid)
Monitor clinical documentation for accuracy, timeliness, and completeness
Conduct case conferences and coordinate care with physicians and interdisciplinary teams
Track patient outcomes and implement quality improvement initiatives
Assist with staff hiring, onboarding, training, and performance evaluations
Manage scheduling and staffing to ensure adequate patient coverage
Address patient concerns, incidents, and clinical issues promptly
Attend field visits to patient homes as needed
Qualifications
Active Massachusetts Registered Nurse (RN) license is required
Masters of Science in nursing preferred
1-2 years of supervisory experience preferred
3 years of home health or related clinical experience
Strong knowledge of home health regulations and reimbursement guidelines
Proficiency in EMR/EHR systems
Proficiency in Microsoft Word, Outlook, and Excel is required
