Albany, Lebanon, Corvallis, Dallas and other areas as needed
Location:
Salem, Oregon (On‑site)
The LPN provides quality care to patients in the nursing center and in their homes under the direction of a Nurse Case Manager or Clinical Supervisor. The duties follow the established nursing process and procedures and are conducted in accordance with the Nurse Practice Act. The role requires compliance with all applicable state, federal, and regulatory body standards. LPNs working in Home Health spend 25% to 35% of their time traveling daily.
Required Qualifications
Education to meet certification, license or registration requirement.
Oregon Licensed Practical Nurse upon hire, or
National Provider BLS – American Heart Association certification within 30 days of hire.
Driving may be necessary; caregivers must provide proof of driver license and auto insurance and comply with all state driving laws.
License in states where care is delivered upon hire.
1 year of experience in Acute Care, Home Health, public health nursing, or patient care in a skilled nursing facility or medical clinic.

Pay Range:
$31.35 - $43.33 per hour. Additional compensation may include shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
