Location:
Pasadena and surrounding areas
Schedule:
Evenings / Nights / Weekends (based on branch needs)
Compensation:

Concho Hearts Hospice , our On-Call Nurses provide comfort, guidance, and skilled care when patients and families need support the most.
This role is ideal for an RN who is calm under pressure, works well independently, and values meaningful patient care. Hospice on-call nursing allows you to make a real difference after hours while being supported by an experienced clinical team.
What You’ll Do:
Respond to after-hours patient and family calls with professionalism and compassion
Triage clinical concerns and determine appropriate interventions
Make urgent visits as needed in patients’ homes or facilities
Provide symptom management and end-of-life nursing support
Coordinate with physicians, case managers, and the interdisciplinary hospice team
Complete timely documentation using the EMR system
Ensure patients and families feel supported, informed, and cared for after hours
What We’re Looking For:
Active Texas RN license
Hospice, home health, ER, ICU, or acute care experience preferred
Strong clinical assessment and triage skills
Ability to work independently and make sound decisionsCompassionate communication skills
Reliable transportation and willingness to travel locally
Current CPR/BLS certification
