Responsibilities

Assist clients in advancing decision-making, productivity, and participation in daily activities.

Facilitate the client’s inclusion in positive community experiences.

Promote independence in daily activities while mindful of the client’s intellectual and physical disabilities.

Assist with meals, cooking, household maintenance, grooming and dressing as assigned.

Maintain current knowledge of client’s needs.

Implement individual training and support programs under the direction of the case responsible person.

Maintain positive communications and relationships with supervisor, clients, landlords, employers, family members, Regional Center, D.O.R., and others.

Participate in evaluation of the client through task analysis and other assessment devices.

Document required activity records, reports, incidents, and client evaluations accurately and timely.

Provide positive advocacy when the individual's preferences, needs, or talents are neglected.

Oversee client activities and environment throughout shift to ensure health and safety.

Provide direct service as needed; transport clients to appointments, community activities, or other locations, ensuring safety and timeliness during travel.

Requirements

Education:

High School Diploma or Equivalent.

Age:

18 years of age or older.

First-Aid / CPR:

Certification in an approved First-Aid and CPR class; or successful completion within 45 days of hire.

Vehicle & License:

Insured, registered vehicle in safe working condition and a current CA driver’s license.

Background:

Successful completion of pre-employment drug screen and background check.

Mobile Device:

Adjoin’s client case management platform requires employees to utilize a mobile smart device with reliable internet connectivity. Employees receive reasonable reimbursement for device availability.

Schedule:

Direct Support Professional works as scheduled by the Regional Manager or designee. Schedule may be part or full time depending on client needs; overtime may be required; no holiday or overtime may be worked without prior supervisor approval.

Travel:

Ability to travel within and throughout counties in which services are provided a minimum of 75% of the time.
