Overview
Social Worker (MSW) Per Diem, Shift/Days Varies, Atlantic Health Morristown Medical Center, Morristown, NJ
Responsibilities
Provide therapeutic intervention and social work services to patients and families to enhance comprehensive, integrated, and uninterrupted hospital care and to support continuity of care in the community.
Assess the patient's psychosocial situation in relation to medical needs and plan appropriate interventions and treatment goals to improve patient/family functioning.
Interview patients, family members, and/or significant others to assess needs for social work and/or interventions related to social determinants of health.
Participate in Multidisciplinary Rounds: Psycho/Social Context; function as resource and advocate for Advanced Care Planning discussions.
Other duties as assigned.
Provide psychosocial support to patients and families during adjustment to illness and life changes.
Collaborate with the health care team regarding in-hospital needs and post-hospital care; maintain knowledge of available psychosocial services and provide relevant information.
Develop and maintain relationships with community organizations; coordinate discharge planning to identify patient needs and post-care services.
Ensure continuity of care through comprehensive discharge planning with patient, family, and/or significant others.
Document psychosocial assessments, individual contacts, family interventions, and discharge planning according to policy and regulatory requirements.
Develop social service networks with city, state or other governmental services to assist patients in obtaining services such as food assistance and transitional housing.
Evaluate psychosocial service program objectives and participate in program planning and improvement in alignment with the hospital mission.
Qualifications
Required: MSW from an accredited school of Social Work; prior hospital social work experience; NJ LSW.
Preferred: LCSW.
Competencies: Excellent communication, negotiation, and conflict resolution skills; knowledge of clinical practice trends, healthcare informatics, regulatory standards related to social work; ability to support rapid-cycle change or clinical improvement.
Schedule
Monday–Friday 8:00 a.m. to 4:00 p.m. with occasional weekend as needed.
