This position offers both part‑time and full‑time options:
Part‑Time: 2 days per week (daytime hours; you pick the days!)
Full‑Time: Monday–Friday (daytime hours, typically a building a day)

Coverage Areas
This role includes 1–2 days in Gatesville, NC, or additional clinical service days in one or more of the following locations:
Charlotte, Mooresville, Salisbury, Clemmons, Lexington, Sparta, Wilkesboro, Henderson, Yanceyville, and Wilmington, NC.
Job Summary
Telos Health Systems is seeking a Psychotherapist (LCMHC, LMFT, Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.
Key Responsibilities
Conduct comprehensive mental health assessments and diagnostic evaluations
Provide individual psychotherapy and ongoing treatment services
Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team
Maintain accurate, timely, and compliant clinical documentation
Coordinate care with facility staff, medical providers, and healthcare professionals
Adhere to all federal, state, and organizational standards, policies, and regulations
Qualifications
Required Licensure
Licensed Clinical Mental Health Counselor (LCMHC)
Licensed Marriage and Family Therapist (LMFT)
Licensed Psychologist (PsyD/Ph.D.)
Additional Requirements
Master’s or Doctoral degree in a relevant mental health field
Active, unrestricted license in the state of practice
Ability to work independently while collaborating within a clinical team
Strong communication and clinical documentation skills
Preferred Experience
Experience working with geriatric populations
Experience in Skilled Nursing Facilities, Assisted Living, or long‑term care settings
