About the Position
The Director of Social Services will identify and provide for each resident’s social, emotional and psychological needs, ensure their full potential development during their stay at the facility, and assist in planning for their discharge.
Essential Duties & Responsibilities
Identify and participate in process improvement initiatives that improve the customer experience, enhance workflow, and/or improve the work environment.
Manage hiring, training, development, coaching, counseling, and, if needed, termination of department staff.
Complete the Social Services portion of the MDS, Resident Assessment Protocols, Resident Care Plan, Progress Notes, Social Services Assessment, and other required forms accurately and in a timely manner to comply with federal and state regulations and facility policies.
Assist residents and families with social, emotional, and family issues.
Participate in resident care planning by identifying the social and emotional needs of the residents in accordance with the medical assessment.
Maintain progress notes for each resident as per company policy and state/federal regulations, reflecting progress relative to the Plan of Care.
Make referrals to support agencies when required by the interdisciplinary team and keep records of such referrals.
Participate in the development of discharge plans and prepare residents for transition.
Serve as a general information source for families, residents and the public.
Visit residents and provide services such as communicating with family or friends, utilizing community resources, and informing residents or legal representatives of Residents’ Rights.
Coordinate logistics and content for monthly Family Council Meetings.
Coordinate resident room changes.
Contact residents or legal representatives to invite them to care plan meetings and participate in resident evaluation.
Maintain contact with residents, legal representatives, and approved family members to involve them in the resident’s total care and provide status information as needed.
Work with Activity Coordinator to provide requested effects for residents lacking family support.
Promote favorable working conditions and relationships with the administration, medical staff, physicians, consultants, other departments, residents, families, volunteers, agencies, and any other affiliated individuals or groups.
Collaborate closely with the facility mental health provider to ensure all resident psychosocial needs are identified, referrals made, and services provided.
Assist in educating the community about aging, residents’ rights, facility services, and related topics.
Orient residents to the long‑term care environment and facilitate adjustment upon placement.
Other special projects and duties as assigned.
Job Requirements
Bachelor’s degree in social work or a human services field (e.g., sociology, special education, rehabilitation counseling, psychology).
One (1) year of supervised social work experience in a health care setting working directly with individuals.
Approvals: if educational degree is other than those listed or if no educational degree, approval from Home Office is required.
Approved social work experience in a health care setting.
Effective verbal and written English communication skills.
