Physical Therapist – Fee for Service; Open in Brooklyn, Queens, and Manhattan.
Overview
Your Choice At Home, an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA), offers 24/7 in‑home care services. In this role you will evaluate clients’ physical functioning and plan and coordinate therapy according to a physician’s prescription.
Responsibilities
Develop a treatment plan that includes modalities, visit frequency, expected outcomes, and short‑ and long‑term goals.
Implement the plan under physician direction and in collaboration with agency staff.
Observe and record patient responses, report changes to the physician and program personnel.
Qualifications
Licensed and currently registered to practice as a Physical Therapist in New York State.
Graduate of an APTA‑approved or AMA Council‑approved physical therapy program.
Minimum one year of experience as a Physical Therapist in a hospital or long‑term care setting.
Experience with pediatric patients required; experience with adult/geriatric patients a plus.
Knowledge of Medicare/Medicaid, regulatory, credentialing criteria, and standards.
Prior home‑care Physical Therapist experience a plus.
Reliable transportation for home visits within assigned service areas.
Salary
$85.00 per visit.
