Responsibilities
Assess and monitor physical, emotional, and psychological needs of patients
Create home health care plans that align with MD orders and patient goals
Administer medications, treatments, and interventions
Provide pain and symptom management
Educate and support patients’ families and caregivers
Collaborate with interdisciplinary team
Maintain accurate and timely documentation
Participate in on-call rotation as required by local branch
Qualifications
Registered Nursing degree (Associate or Bachelor) with current unrestricted license in applicable state
Minimum one year nursing experience in a patient care setting; home health, geriatrics or related preferred
Valid driver's license with acceptable driving record and proof of car insurance in accordance with policy
CPR certification
New nursing graduates may be considered in select markets based on program availability
