Wolf Creek Care Center is seeking a dedicated Physical Therapy Assistant (PTA) to join our dynamic therapy team. We are committed to providing high‑quality, resident‑centered care in a supportive and collaborative skilled nursing environment.
Position Summary
As a PTA, you will work hands-on with patients to implement physical therapy plans designed by licensed Physical Therapists. You'll play a key role in helping patients recover from illness, surgery, or injury, promoting mobility, function, and quality of life.
Responsibilities
Provide direct patient care under the supervision of a licensed Physical Therapist
Document patient progress and communicate updates to the therapy team
Collaborate with nursing, therapy, and medical staff to ensure coordinated care
Ensure compliance with safety, infection control, and facility policies
Maintain compliance with state and federal regulations and professional standards
Qualifications
Associate degree from an accredited Physical Therapist Assistant program
Active California PTA license
Strong communication, teamwork, and documentation skills
Prior SNF experience preferred but new grads are welcome to apply
