Overview
The Acute Rehab Clinical Liaison is responsible for generating admissions to Acute Rehab, through direct contact with external referral sources, including Princeton Healthcare System. Screens referrals for level of care and collaborates with physicians, financial counseling, and social work to coordinate appropriate care levels. The position can be held by an RN, PT, OT, or SLP licensed professional.

Responsibilities

Screens outside and inside referrals from physicians, case managers, social workers, family members, etc. for potential admission to the Acute Rehabilitation Unit.

Evaluates referrals based on the CMS 13 (Medicare) admission criteria, including clinical condition, mental and psychosocial needs, level of care, equipment needs, potential for discharge, and financial status taking into consideration the 60/40 CMS rule.

Evaluates referrals based on chart reviews, discussions with the acute rehab management team and physician, and meetings with patient and patient family.

Completes all pre‑admission assessments and maintains files.

Enhances relationships with existing referral sources and develops new relationships with physician groups and outside facilities; presents information about Acute Rehab to community groups, managed care organizations, hospitals, assisted living communities.

Coordinates daily admissions decisions at the daily census meeting and assures complete admission assessments are available for the meeting.

Communicates the results of admissions decisions to referral sources and continues communication with patients as appropriate.

Works effectively with the Case Management team to expedite patient referrals to community or facility‑based services in alignment with cost‑effective and appropriate care goals.

Completes all work timely and establishes a minimum 24‑hour return‑call policy to referral sources.

Supports customer service and performance improvement goals, and provides referral/admission statistics to clinical staff as needed.

Communicates pre‑admission clinical needs to nursing, including setup days for dialysis, radiation therapy, O₂, vacs, TPN, etc.

Meets departmental admission objectives in both census and census mix.

When necessary, visits outside referring facilities to determine appropriateness of referrals and clinically assess patient suitability.

Assists in development of a marketing plan and identification of potential referral sources.

Meets established referral, admission, and census targets.

Participates in marketing lunches, dinners, and in‑house marketing meetings.

Actively participates in all CARF‑related activities.

Credentials

Basic Cardiac Life Support (Required)

Driver's License (Required)

RN, PT, OT, or SLP (Required)

Licensed Registered Nurse in the State of NJ or licensed PT, OT, SLP (Required)

Education / Equivalent Experience

Associate of Arts or Science (Required)

Bachelor of Arts or Science (Preferred)

1+ years of admission experience in Acute Rehabilitation (Preferred)

Specific experience working with Medicare skilled‑care criteria, Medicaid, managed care, and private pay.

Salary
$78,265.82 - $124,813.73 Annually, based on 40 hours per week.
