Responsibilities
As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at
Bella Terra La Grange
by ensuring social and emotional needs are met during their stay, coordinating resident discharges, and participating in resident care conferences. Your knowledge, skills, and attention to detail will help keep our residents comfortable and safe.
Qualifications
To be eligible for consideration applicants should have: a high school diploma or equivalent and two (2) years of experience in a Social Worker role, preferably in a healthcare setting. An associate degree in Social Work, Social Services Certificate, or equivalent field is preferred, but we are willing to invest in the right candidate.
