Psychiatric Mental Health Nurse Practitioner (PMHNP)

to provide high-quality

geriatric psychiatry and behavioral health services

in home, assisted living, memory care, and skilled nursing settings.

senior mental health, dementia care, integrated care models, and autonomous practice , this is an opportunity to build and manage your own panel while collaborating with primary care, palliative care, and neurology teams.

Key Responsibilities

Comprehensive psychiatric evaluations & medication management

Management of behavioral symptoms in dementia and cognitive decline

Collaboration with PCPs, palliative care, and facility partners

Caregiver education and relationship-based care

Documentation, care coordination, and panel management

What We’re Looking For

Active

PMHNP-BC license

Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry

Strong diagnostic and medication management skills

Ability to practice independently while contributing to a team-based model

Leadership potential with interest in growing a successful patient panel

Pay Range for Posted Region: $110,000 - $150,000 USD
