Social Worker

Job Summary
We are seeking a dedicated and compassionate Social Worker for a full-time, on‑site position based in Huntingdon Valley, PA. The Social Worker will provide emotional support, counseling, and assistance to patients and their families, connecting them with resources and services that address their social, emotional, and healthcare needs.

Responsibilities

Conduct assessments of patients and families

Develop individual care plans

Collaborate with interdisciplinary teams to advocate for patient welfare

Provide crisis intervention as needed

Facilitate communication between patients and healthcare professionals

Qualifications

Strong interpersonal, communication, and counseling skills to support patients and families effectively

Proficiency in conducting assessments, developing care plans, and coordinating services

Understanding of healthcare systems, community resources, and crisis intervention strategies

Ability to work empathically and collaboratively with diverse populations

Organizational and time‑management skills to handle casework efficiently

Master's degree in Social Work (MSW) from an accredited program

State licensure or eligibility for licensure as a Social Worker in Pennsylvania

Experience in a healthcare, clinical, or social services setting is highly desirable

Ability to navigate sensitive and confidential information with professionalism
