Part-time / Day Shift
Responsibilities
Enrich lives as a nursing center Social Services Specialist working with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and obtain services to meet those needs.
Coordinate services to provide the highest practical level of physical, mental, and psychosocial well‑being and quality of life, in accordance with NASW Code of Ethics and federal, state, and local regulations.
Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well‑being, educating staff on the role of social services.
Participate in the development of a written interdisciplinary plan of care for each patient/resident to address social service needs.
Hold meetings with patients/residents, families, and significant others to provide support and information that improves quality of life.
Perform discharge planning activities including patient/family education, nursing center documentation, and referrals to appropriate agencies and suppliers.
Establish and maintain relationships and referral flow with community‑based agencies/services that benefit the patient/resident population.
Qualifications
Bachelor's degree in social work or human services.
Must possess any certifications/licensures required by the State of employment to practice in long‑term care.
