Floating RN Case Manager
to support our growing service areas in the Greater Twin Cities. This full-time position offers variety, the consistency of 1:1 home care, and strong team support.

Diverse experience:
Support a range of clients and care teams across the metro
High impact:
Step in to stabilize caseloads and ensure consistent, high-quality care
Autonomy + teamwork:
Work independently while staying connected to a responsive clinical leadership team
Incentive pay:
Earn additional compensation for visits in support of fluctuating census or expanded territory needs
Key Responsibilities
Deliver skilled nursing care and perform comprehensive client assessments
Develop and update individualized care plans
Provide caregiver education and support to maintain quality standards
Rotate into the
on-call schedule
Travel as needed to different areas of the Twin Cities metro to assist with coverage
Qualifications
Active RN license
in the State of Minnesota
Reliable transportation
and valid insurance (position involves regular travel)
Strong clinical judgment and communication skills
Experience in home health is preferred , but not required—we’re happy to train compassionate, motivated RNs
