is seeking an
Optometrist
to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health.
Responsibilities
Perform comprehensive eye examinations and vision assessments
Diagnose and manage common eye conditions and vision disorders
Prescribe corrective lenses and contact lenses as appropriate
Identify and refer patients with complex or surgical needs to specialists
Educate patients on eye health, preventive care, and treatment plans
Document all encounters accurately in the electronic health record (EHR)
Qualifications
Doctor of Optometry (OD) degree from an accredited program
Current New York State optometry license (or eligibility)
Experience in outpatient or community health settings preferred
Strong communication and patient engagement skills
