Main Purpose of Job
The Developmental Disabilities Program (DDP) TCM/Coordinator of Community Services provider is the point person for all services offered to individuals served by DDA. The CSP I, HS provides coordination of available community resources for eligible individuals with developmental disabilities who are residents of Maryland’s Eastern Shore. The CCS assesses and determines eligibility for DDA services, provides the referral for those services, arranges access to day/residential and other support services, facilitates annual planning meetings, assists the served individuals and their families with developing a plan of service, conducts periodic monitoring of satisfaction with delivered services, follows individuals on the waiting list registry, and assists in transition planning. This position is responsible for the development of the Person Centered Plan (PCP) and subsequent monitoring of the PCP. This position also is accountable for accurate documentation of all services rendered. These efforts work to maintain the supported individuals in the community and to promote individual choice and overall quality of life.

Qualifications
Education:

This position requires:

A Bachelor's degree from an accredited college or university in a Human Services field; OR

An Associate’s degree with two (2) years experience in a Human Services field; OR

Seven (7) years experience in a Human Services field.

Minimum Qualifications:

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

Desired or Preferred Qualifications

Experience with individuals that have intellectual or developmental disabilities.

Experience in case management.
