Responsibilities

Assessment

Collects and organizes patient data using all available resources.

Examines patient to determine physical status including interview, observation, and inspection.

Identifies and records patient’s physical, psycho‑social condition including history, health habits, and patient perception.

Planning

Formulates a care plan identifying patient needs in priority order, including patient and/or family participation, and expected outcomes.

Collaborates with the health team.

Identifies patient educational needs and coping mechanisms.

Anticipates workflow and completes tasks.

Responds to schedule changes and/or emergencies.

Implementation

Delivers care that honors patient dignity.

Adapts nursing procedures when possible to meet individual patient needs.

Provides daily hygiene and acceptable appearance.

Involves patient and family in care when it is therapeutic.

Identifies physical symptoms and changes and takes appropriate action.

Administers medications and treatments following established policies.

Observes behavioral and physiological changes due to medications and acts appropriately.

Uses asepsis and necessary equipment for delivery of care.

Helps patient accept appropriate dependence/independence.

Protects patients from environmental hazards.

Ensures adequate hydration and elimination.

Responds to anxiety or distress with appropriate action.

Provides mental and social stimulation on a suitable basis.

Implements teaching plan using principles of teaching and learning.

Evaluation

Uses current data to measure progress toward goal achievement.

Initiates nursing actions when patient fails to progress toward pre‑set goals.

Evaluates teaching by obtaining patient or significant other’s verbalization of instructions.

Refers unmet needs to appropriate personnel and obtains feedback.

Documentation

Writes admission notes reflecting a clear picture of patient’s physical and psychological status.

Records data on the Nursing Care Plan.

Writes nurses notes indicating progress or lack thereof.

Uses graphic/flow sheets appropriately.

Writes discharge notes reflecting patient’s status at discharge.

Professional Implications

Makes decisions reflecting sound judgment based on facts.

Suggests constructive changes in delivery or monitoring of patient care.

Participates in review and revision of policies and procedures as directed.

Serves on committees.

Enforces Universal Precautions.

Performs related duties as required.

Qualifications

Graduate from an accredited School of Nursing. Bachelors Degree in Nursing preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010, and obtain a BSN within five (5) years.

Current license to practice as a Registered Professional Nurse in New York State.

Acceptable for ADA essential functions.

Salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant. Compensation may vary based on location, specialty, service line, years of experience, education, credentials, negotiated contracts, budget and internal equity.
